As a condition of your enrollment at California State University, Fresno you will be automatically enrolled in the Fresno State-approved health insurance policy when residing in the US.

Those students who have coverage provided by a sponsoring organization, such as the Saudi Arabian Cultural Mission, will not be required to enroll in the Fresno State-approved health insurance plan. Please purchase travel insurance ONLY for the period that you will be in the United States that is not covered by the effective dates of the policy listed above.

All incoming new students are required to provide specific health information to the Fresno State Student Health & Counseling Center for health clearance. All incoming students starting in or after Fall 2021 will need to submit proof of immunizations in accordance with CSU Executive Order 803. You must submit proof of your immunizations before you will be allowed to register for your second semester.
Please review the Immunization Requirements for International Students along with the directions on how to upload your Immunization records (this is the new link: http://fresnostate.edu/studentaffairs/health/medical/immunizations.html).
Note: The University will only accept immunization documentation via the online portal.
You will need the following documents to complete your health clearance requirements:
- All immunization records from childhood and adulthood.
- Any blood test results for Tuberculosis (TB) and chest x-ray reports if you have them.
- Please complete this TB Blood Test Verification Form and bring supporting documents to the Check-In Days. A representative from the Student Health & Counseling Center will be present to further assist you and answer any of your questions.

Your Anthem Student Advantage Medical ID Card will NOT available online until your insurance start date of 8/9/2021. If you are a late transfer/enrollee at Fresno State, your Medical ID Card will not be available until 2-3 weeks after your enrollment in the plan!
Anthem Student Advantage Medical ID Cards are electronic only. You must visit Anthem’s website: www.anthem.com/ca or download the Sydney Health Mobile App to access your card.
You will be able to print your Medical ID Card from the Anthem site or save a copy of it to your smartphone.
Show your Medical ID Card whenever you receive medical treatment.
Health Insurance does NOT COVER dental, vision, spouse, or dependents.

If you receive a bill directly from a doctor’s office or hospital, please call Anthem to ensure they have received it as well.
If you use the Fresno State Health Center, save your receipts!! You will have to pay the cost upfront and then you should refer directly to Anthem for claims/bills assistance.
